Password Assistance
If you have forgotten your password, below are the ways to get help:
- If you have secret questions set up on your account, then you can use them to self-reset your password. Use the Forgot My Password link on the OneHealthPort website.
If you are unable to reset your password using your secret questions, please contact your organization's OneHealthPort account Administrator for assistance. Your Administrator is the only person who can reset your password.
Administrators: Log in to your Administrator Account for the password reset function.
- If you don't know your organization's OneHealthPort Administrator, or you are the only Administrator for your organization, fill out the contact form for assistance.

If you need help reactivating your OneHealthPort account, using Multi-Factor Authentication, or have any login-related questions, please call 1.800.973.4797 or fill out the contact form.
Note: OneHealthPort cannot answer questions about eligibility, benefits, or contracting status. For these inquiries, you must contact the health plan directly.
